We can guess that the unacceptable conduct of the soldiers at Abu Ghraib resulted in part from the dangerous state of affairs on the ground in a theater of war.

The CIA teamed up with Army, Air Force and Naval Intelligence to run one of the most nefarious, classified, enhanced interrogation programs of the Cold War. The work took place inside a clandestine facility in the American zone of occupied Germany, called Camp King.

Military intelligence interrogators, however, their goal is to get information, to save lives, to stop the war, to find Saddam - whatever the information is going to be used for, at whatever cost.

I don't know anyone at the highest levels who approved Abu Ghraib. If President Barack Obama for a moment thought that somebody at a high level had approved it, he would go after them.
